Tellingly, the in-game note clarifies it as "reach the 99 day mark for the first time," meaning surviving the full 99-night gauntlet is what unlocks it. It is the only survival badge bumped up to 3 difficulty stars, a fair signal that almost no run reaches this far without near-flawless play.
Earning it is the culmination of everything the lower milestones taught: an over-stocked, fortified base, redundant fires, repaired top-tier weapons, and a team holding strict gather, cook and defense roles through the harshest late-game nights. The final stretch past day 80 is where complacency kills colonies, so the rule is simple, never gamble. Keep an untouched emergency surplus, refuse risky night expeditions, and protect every survivor as if the run depends on them, because at this point it does.
